Where the Fat Is Matters More Than How Much

Waist-to-hip ratio measures fat distribution rather than quantity. It matters because visceral fat — the fat packed around the abdominal organs — is metabolically active in ways subcutaneous fat is not. It releases inflammatory cytokines and free fatty acids directly into the portal circulation, driving insulin resistance and cardiovascular risk.

Two people with an identical BMI can therefore carry very different risk, and the INTERHEART study found waist-to-hip ratio predicted heart attack substantially better than BMI across 52 countries.

Apple and Pear

An apple shape stores fat abdominally and carries the higher metabolic risk; a pear shape stores it around the hips and thighs and carries less. The distinction is largely driven by sex hormones and genetics, and it is not something exercise can redirect — spot reduction does not work, though overall fat loss reduces visceral fat preferentially.

Waist-to-Height Is Simpler and Just as Good

Keeping your waist under half your height is a single rule that works across sexes, ethnicities and ages, including children. It requires one measurement rather than two and performs comparably to WHR in predicting cardiometabolic risk. For a 178 cm person, that means a waist under 89 cm.

Measuring Properly

  • Waist — at the narrowest point between the ribs and hips, or at the navel if there is no clear narrowing. Measure after breathing out normally, without pulling the tape tight.
  • Hips — at the widest point of the buttocks, with feet together.
  • Measure in the morning before eating, over bare skin, and take the average of two attempts.

Frequently Asked Questions

Can I change my waist-to-hip ratio?

Yes, by losing fat overall. Visceral fat is the first to respond to a calorie deficit and to aerobic exercise, so waist measurement usually improves faster than total weight.

Is a very low ratio a problem?

Not in itself. Very low ratios usually reflect an athletic or naturally narrow-waisted build. The thresholds identify elevated risk at the upper end, not an optimum.

Which is better, WHR or waist alone?

Waist circumference alone performs nearly as well and is simpler to track over time. WHR adds information mainly for people at the extremes of body shape.
