The Best Single Measure of Fitness

VO2 max is the maximum volume of oxygen the body can use per minute, expressed per kilogram of body weight. It sets the ceiling on sustained aerobic performance, and it is one of the strongest predictors of all-cause mortality in the epidemiological literature — stronger than smoking status, hypertension or diabetes in several large cohorts.

Values decline roughly 10% per decade after 30 in sedentary people, and about half that in those who keep training. Much of the apparent age-related decline is disuse rather than ageing.

Estimating It Without a Laboratory

  • Cooper test — the furthest distance run in 12 minutes. VO2 max = (metres − 504.9) / 44.73. Simple and reasonably accurate for anyone able to run hard for 12 minutes.
  • Resting heart rate method — 15.3 × (max HR / resting HR). Requires no exercise at all and is correspondingly rougher.
  • Race time — the Daniels and Gilbert equations derive VO2 max from any race between 1,500 m and the marathon. The most accurate non-laboratory method for trained runners.

A true measurement requires a graded exercise test with a metabolic cart — running to exhaustion while breathing through a mask.

What Improves It

The most effective stimulus is training near VO2 max intensity: intervals of 3–5 minutes at roughly 90–95% of maximum heart rate, with equal recovery. Four to six repetitions, twice a week, produces measurable improvement in six to eight weeks.

High-volume low-intensity training matters too. It builds the mitochondrial density and capillary network that support the ceiling, which is why elite programmes combine large amounts of easy work with a small amount of very hard work.

The Genetic Ceiling

Baseline VO2 max is substantially heritable, and trainability itself varies — the HERITAGE Family Study found improvements ranging from zero to over 40% in response to identical training. This is worth knowing so that a modest response is not read as a failure of effort.

How accurate are watch estimates?

Modern devices using heart rate and pace typically land within 5–10% of laboratory values for running, and are less reliable for other activities. They track change over time better than they measure absolute values.

Does losing weight improve VO2 max?

Arithmetically, yes — it is expressed per kilogram, so losing fat raises it without any change in oxygen uptake. This is real for performance, since you are carrying less weight.

What is a good VO2 max for my age?

Anything above the average for your age group is a reasonable target, and the health benefit of moving out of the bottom quintile is far larger than the benefit of moving from good to excellent.
